Assistance: Health assessment, consultation, counseling and coordination of health services are provided daily by our nurse. If needs change, the extra support and assistance you may require will be provided from the comfort of your own home.

Nursing Home: Should more extensive medical services be required, our  Sweet Brook Transitional Care and Living Center, our sister facility, is located immediately adjacent to Sweetwood, and provides nursing and physical therapy services.
